Patient Terms of Use

1. BridgeCare LA Services
These Patient Terms of Use (the “Terms”) constitute a binding agreement governing the clinical monitoring services provided to you by BridgeCare Louisiana, LLC (“BridgeCare LA,” “we,” “us,” or “our”). Our services may include, depending upon your eligibility and the direction of your ordering physician, remote physiologic monitoring (“RPM”), chronic care management (“CCM”), advanced primary care management (“APCM”), and other clinical status monitoring programs (collectively, the “Services”) delivered through the SYNC-PREVENT™ platform, developed and licensed by Synchronize Health, LLC.
The Services involve clinical status monitoring, which may include coordinating and monitoring your health data, facilitating communication between you and your healthcare providers, and supporting the development of care plans based on biomarker data, FDA-cleared device readings, and longitudinal patient records. The Services may also include providing you with connected medical devices (as defined in Section 6 below) that you will use to transmit physiologic readings to BridgeCare LA for non-continuous monitoring by a clinical care team. By using the Services, you agree to comply with these Terms and any instructions provided to you by BridgeCare LA or your ordering physician. You understand that you may disenroll from the Services at any time. You agree that the Services begin upon your consent (verbal or written) to participate.
2. Non-Emergency Use
Important: The Services are not intended for continuous monitoring of any health condition and do not enable immediate clinical action in an emergency. The Services are not an emergency response system. If you are experiencing a medical emergency, you should call 9-1-1 or go to the nearest emergency room immediately. Do not rely on the Services to have your health conditions continuously monitored in real time.
If at any time you are concerned about your care or treatment, or you believe you may have a serious or life-threatening condition, seek appropriate emergency care immediately. YOU AGREE THAT IF YOU USE THE SERVICES DURING OR IN RELATION TO AN EMERGENT, SERIOUS, OR LIFE-THREATENING CONDITION, SUCH USE IS AT YOUR OWN RISK.
3. Physician Authority
All Services are provided under the direction of an ordering physician. The ordering physician retains full clinical authority over all treatment decisions at all times. BridgeCare LA provides clinical status monitoring, data interpretation, and care coordination support. BridgeCare LA does not independently diagnose, treat, or prescribe. Any clinical recommendations generated through the SYNC-PREVENT™ platform are subject to physician review and approval before any action is taken.
4. Patient Responsibilities
To receive the Services, you must remain engaged, which may include but is not limited to:
- Attending and participating in scheduled telephone or telehealth appointments
- Responding to communications from BridgeCare LA
- Transmitting your physiologic data from assigned devices as directed
- Providing complete and accurate personal health information
- Treating our staff respectfully at all times
BridgeCare LA may terminate your Services if you fail to comply with the foregoing, fail to communicate with us for at least 60 days, or fail to comply with these Terms. Any clinical guidance provided to you is based on the health information you provide; if you do not provide complete and accurate information, the guidance you receive may not be appropriate. If applicable, you agree that we may share information about you and our Services with any of your appointed representatives and/or caregivers.
5. Financial Obligations
YOU AGREE TO BE FINANCIALLY RESPONSIBLE FOR THE SERVICES. DEPENDING UPON YOUR INSURANCE COVERAGE, YOU MAY BE RESPONSIBLE FOR PAYING ANY COPAYMENT, COST-SHARING, CO-INSURANCE, OR DEDUCTIBLE AMOUNT ASSOCIATED WITH THE SERVICES. YOU UNDERSTAND THAT THERE MAY BE SEPARATE CHARGES ASSOCIATED WITH ENROLLMENT IN RPM, CCM, APCM, AND/OR OTHER PROGRAMS.
You assign and authorize payment of any third-party benefits you may have, such as insurance coverage, to us and/or your ordering physician, as applicable, and agree to be financially responsible for the Services. BridgeCare LA is not an insurance company or medical billing company and cannot guarantee your financial obligations in any instance. You understand that only one provider may furnish RPM, CCM, and APCM services to you at any given time.
6. Devices
The Services may be offered in conjunction with FDA-cleared medical devices, which may include blood pressure monitors, continuous glucose monitors, weight scales, pulse oximeters, hemodynamic monitoring devices, foot temperature sensors, inhaler technique monitors, and other connected devices (the “Devices”) that measure certain physiologic data and transmit that data to BridgeCare LA through the SYNC-PREVENT™ platform. You are responsible for:
- Using the Devices appropriately and in accordance with instructions provided by BridgeCare LA and the device manufacturer
- Not transmitting data of anyone other than yourself
- Remaining engaged in the Services by taking your readings as directed
- Returning Devices in good condition (excluding normal wear and tear) upon completion of Services
BridgeCare LA does not manufacture the Devices provided with the Services and is not responsible for any inaccuracies or transmission delays that may occur when you use a Device or when data is transmitted to the SYNC-PREVENT™ platform.
7. Communications
You agree that BridgeCare LA may communicate with you through phone calls (to your landline or cellular number), voicemails, text messages, and email, including through the use of automated telephone dialing systems and artificial or prerecorded voice messages. These communications may concern any matter related to your care, including but not limited to medical treatment, device readings, prescriptions, insurance eligibility, scheduling, billing, or collection matters.
You understand that these communications may not be encrypted or secure, and you assume the risks of transmitting health information via unsecure means. You understand that calls between you and/or your caregiver and BridgeCare LA may be recorded as permitted under applicable state law for quality assurance purposes. If you incur any costs from being contacted at the telephone number(s) or email address(es) you have provided, including data, roaming, text message, or other fees, you understand that you are responsible for such costs.
8. HIPAA
BridgeCare LA is required to protect the confidentiality and security of your Protected Health Information (as defined by the Health Insurance Portability and Accountability Act of 1996, as amended, “HIPAA”) in accordance with HIPAA. As permitted by HIPAA, you agree that in providing the Services, BridgeCare LA may use and disclose your Protected Health Information for payment, treatment (including coordination between healthcare providers, referrals, follow-ups after emergency visits, and post-discharge care), and healthcare operations purposes, and to any individuals or entities that you instruct may receive your Protected Health Information (such as your caregivers and/or emergency contacts). 9. Disclaimer of Warranty; Limitation on Liability
THE SERVICES ARE PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTY OF ANY KIND, EXPRESS OR IMPLIED. BRIDGECARE LA, ITS AFFILIATES, LICENSORS, AND SUPPLIERS DISCLAIM ALL EXPRESS AND IMPLIED WARRANTIES WITH REGARD TO THE SERVICES, INCLUDING ANY INFORMATION AND MATERIALS PROVIDED THROUGH THE SERVICES, INCLUDING WITHOUT LIMITATION ANY IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. YOUR USE OF THE SERVICES IS AT YOUR OWN RISK.
NONE OF BRIDGECARE LA, SYNCHRONIZE HEALTH, LLC, THEIR AFFILIATES, SUPPLIERS, OR ANYONE ELSE INVOLVED IN CREATING, PRODUCING, OR DELIVERING THE SERVICES SHALL BE LIABLE FOR ANY DIRECT, INDIRECT, INCIDENTAL, SPECIAL, EXEMPLARY, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF YOUR USE OF, OR INABILITY TO USE, THE SERVICES. YOU ACKNOWLEDGE AND AGREE THAT THE LIMITATIONS SET FORTH ABOVE ARE FUNDAMENTAL ELEMENTS OF THIS AGREEMENT AND THE SERVICES WOULD NOT BE PROVIDED TO YOU ABSENT SUCH LIMITATIONS. SOME JURISDICTIONS MAY NOT ALLOW THE EXCLUSION OF IMPLIED WARRANTIES, SO SOME OF THE ABOVE EXCLUSIONS MAY NOT APPLY TO YOU.
10. Force Majeure
In no event shall BridgeCare LA be responsible or liable for any failure or delay in the performance of the Services arising out of or caused, directly or indirectly, by circumstances beyond its reasonable control, including without limitation acts of God, earthquakes, fires, floods, wars, civil or military disturbances, acts of terrorism, sabotage, strikes, epidemics or pandemics, riots, power or internet failures, computer or software failure, device malfunction, cellular or cloud service interruptions, accidents, labor disputes, acts of civil or military authority, governmental actions, or inability to obtain labor, material, equipment, or transportation.
11. Intellectual Property
SYNC-PREVENT™ is a trademark of Synchronize Health, LLC. All content, software, and technology associated with the Services are the property of BridgeCare Louisiana, LLC and/or Synchronize Health, LLC and are protected by applicable intellectual property laws. You may not reproduce, distribute, modify, or create derivative works from any materials associated with the Services without prior written consent.
12. Governing Law
These Terms shall be governed by and construed in accordance with the laws of the State of Louisiana, without regard to its conflict of law provisions. Any dispute arising under these Terms shall be subject to the exclusive jurisdiction of the state and federal courts located in Louisiana.
13. Changes to These Terms
We reserve the right to modify these Terms at any time. Changes will be posted on this page with a revised effective date. Your continued use of the Services after changes are posted constitutes your acceptance of the updated Terms.
