HIPAA Notice

BridgeCare Louisiana, LLC (“BridgeCare LA”) takes the protection of health information seriously. This notice describes how we approach HIPAA compliance in our clinical monitoring operations and clarifies the distinction between this website and the clinical services we deliver through the SYNC-PREVENT™ platform.
1. This Website Does Not Collect PHI
This website (bridgecarela.com) is a business-to-business and patient-information site. Any contact inquiries submitted through this site collect business information only. This website does not collect, store, or transmit protected health information (“PHI”) as defined under HIPAA.
2. Our Clinical Operations Are HIPAA-Compliant
When BridgeCare LA delivers clinical monitoring services through the SYNC-PREVENT™ platform under the direction of an ordering physician, we operate as a Covered Entity under HIPAA. This means:
Business Associate Agreements (BAAs) are executed with every business associate and subcontractor before any PHI is accessed or processed on our behalf.
Administrative, physical, and technical safeguards are in place to protect the confidentiality, integrity, and availability of PHI in accordance with the HIPAA Security Rule.
Minimum necessary standard: our team accesses only the PHI required to perform the specific clinical services authorized by the ordering physician.
Workforce training: all team members who may access PHI receive HIPAA training and are bound by confidentiality obligations.
3. Notice of Privacy Practices

4. Breach Notification
In the event of a breach of unsecured PHI, BridgeCare LA will notify affected individuals and, where applicable, business associate partners in accordance with the HIPAA Breach Notification Rule (45 CFR §§ 164.400–414) and will cooperate fully in any required notification to the Department of Health and Human Services.
5. Scope and Clinical Authority
BridgeCare Louisiana, LLC operates clinical monitoring programs under the direction of ordering physicians. All clinical programs comply with applicable federal and state regulations, including HIPAA, and protected health information is handled in accordance with the HIPAA Privacy and Security Rules. BridgeCare LA does not independently diagnose, treat, or prescribe; the ordering physician retains full clinical authority over all patient care decisions. SYNC-PREVENT™ is a trademark of Synchronize Health, LLC, of which BridgeCare LA is the exclusive licensee for the state of Louisiana.
