Who We Are

A Louisiana company doing one thing carefully.

BridgeCare Louisiana is the only company licensed to operate SYNC-PREVENT™ in the state. The platform was built by Synchronize Health, and it does something fairly simple to describe and fairly hard to do well: it pulls a patient's biomarker results, readings from FDA-cleared devices, and their longer medical history into one picture, then keeps that picture current instead of letting it go stale between visits.

We run eight clinical programs, covering heart failure, continuous glucose monitoring, chronic care management, diabetic foot ulcer prevention, inhaler technique, rheumatoid arthritis, ostomy aftercare, and post-TAVR recovery. Different conditions, same discipline behind each one — watch closely, catch the change early, and get it in front of the physician who is actually treating the patient.

That last part is worth being clear about. We do not diagnose and we do not direct treatment. The ordering physician keeps full clinical authority over every decision. Our job is to make sure they are never deciding in the dark.

Certified Diabetes Care and Education Specialist (CDCES)

$80,000–$90,000 / year

Primary Responsibilities

  • Provide individualized Diabetes Self-Management Education and Support (DSMES).
  • Educate patients with Type 1 diabetes, Type 2 diabetes, and prediabetes.
  • Train patients on Continuous Glucose Monitoring (CGM) systems, including device setup, proper use, interpretation of glucose trends, and troubleshooting.
  • Review CGM and glucose data to identify opportunities to improve glycemic control and Time in Range.
  • Educate patients on insulin administration, injection techniques, glucose monitoring, medication adherence, and diabetes self-care.
  • Teach patients to recognize, prevent, and appropriately respond to hypoglycemia and hyperglycemia.
  • Counsel patients on nutrition, physical activity, weight management, and healthy lifestyle modifications.
  • Reinforce physician treatment plans and evidence-based diabetes care guidelines.
  • Conduct scheduled virtual patient visits and follow-up education sessions.
  • Coordinate care with physicians, pharmacists, dietitians, nurses, and care coordinators.
  • Document patient encounters, education provided, care plans, and progress within the electronic medical record (EMR).
  • Escalate significant clinical concerns, abnormal glucose patterns, medication issues, and other patient care needs to the supervising provider.

Qualifications

  • Current Certified Diabetes Care and Education Specialist (CDCES) credential.
  • Current RN, RD, RPh, NP, PA, or other qualifying healthcare licensure.
  • Minimum two years of diabetes education or chronic disease management experience preferred.
  • Experience with Continuous Glucose Monitoring (CGM) systems preferred.
  • Excellent patient education, communication, and motivational interviewing skills.
  • Experience documenting clinical encounters within an electronic medical record.
  • Strong organizational and computer skills.
  • Ability to work independently in a fully remote environment while collaborating with a multidisciplinary clinical team.

Primary Responsibilities

  • Provide individualized Medical Nutrition Therapy (MNT) using evidence-based clinical guidelines.
  • Perform comprehensive nutrition assessments, including evaluation of dietary habits, medical history, laboratory values, medications, and lifestyle factors.
  • Develop personalized nutrition care plans that align with physician treatment goals and patient health objectives.
  • Educate patients on nutrition management for diabetes, hypertension, cardiovascular disease, chronic kidney disease, obesity, hyperlipidemia, and other chronic conditions.
  • Counsel patients on meal planning, portion control, carbohydrate management, label reading, weight management, and healthy food choices.
  • Provide culturally appropriate nutrition education that promotes long-term lifestyle changes and improved health outcomes.
  • Reinforce medication adherence through nutrition education and healthy lifestyle counseling.
  • Conduct scheduled virtual nutrition consultations and follow-up visits.
  • Collaborate with physicians, pharmacists, diabetes educators, nurses, and care coordinators to provide comprehensive patient care.
  • Document nutrition assessments, interventions, education, care plans, and patient progress within the electronic medical record (EMR).
  • Escalate significant nutritional or clinical concerns to the supervising provider as appropriate.

Primary Responsibilities

  • Develop supportive relationships with patients through scheduled telephone and virtual interactions.
  • Provide education regarding chronic disease management, preventive care, medications, and healthy lifestyle habits within established protocols.
  • Encourage patient participation in recommended care plans, screenings, and follow-up appointments.
  • Coordinate communication between patients, providers, specialists, pharmacies, and community resources.
  • Identify barriers to care and assist patients in accessing appropriate services.
  • Document all patient interactions accurately in the electronic medical record (EMR).
  • Monitor assigned patient work queues and follow established outreach schedules.
  • Escalate clinical concerns promptly to licensed clinical personnel.
  • Assist with appointment scheduling, referrals, and care plan follow-up.
  • Maintain HIPAA compliance and protect patient confidentiality.

Qualifications

  • Associate degree preferred; equivalent healthcare experience considered.
  • Experience in patient engagement, care coordination, customer service, healthcare administration, medical office, insurance, or population health preferred.
  • Excellent verbal and written communication skills.
  • Strong organizational and time-management abilities.
  • Comfortable using EMRs and Microsoft Office applications.
  • Ability to work independently in a remote environment.
  • Bilingual (English/Spanish) is a plus.

Preferred Background

  • Candidates with experience as a Patient Service Representative, Medical Assistant, Certified Nursing Assistant (CNA), Licensed Practical Nurse (LPN), Care Coordinator, Health Coach, Medical Scheduler, Case Management Assistant, or Customer Care Representative are encouraged to apply.

  • Fixed and rotating schedules are offered; specific shift assignments are set during onboarding based on program needs and coordinator availability.
  • 100% remote — worked from home through our secure, centrally managed monitoring systems.

Qualifications

  • Healthcare coordination, medical office, CNA, Medical Assistant, or similar experience preferred.
  • Excellent communication and organizational skills.
  • Comfortable using EMRs and Microsoft Office.
  • Ability to work independently in a remote environment.
  • Willingness to work non-traditional hours, including evenings, overnights, weekends, or holidays, as part of 24/7 coverage.
